A research group at HVV carried out follow-up research of an organization-wide intervention in collaboration with Eskilstuna Municipality, Region Sörmland and MKHV. An integrated multi-professional team has designed new ways of working that offer a coherent health and care system for older people in ordinary housing with coordination needs between different care providers.
