According to Swedish law, a coordinated individual planning (CIP) should be offered to individuals who need joint action from social services and healthcare. The basic idea is that CIP will contribute to a coherent support with a high level of individual participation. CIP refers to the process of coordinating individual support based on personal goals that includes decisions and choices about what support the person will get and how support will be given. This means that different professions from different organizations, with different approaches and methods, should work together to provide opportunities for individual participation. The knowledge of participation in individual planning in a collaborative context needs to increase.
